In Jurassic Park when Lex falls through the ceiling, the stunt double accidentally looks up, so instead of filming the scene again, they superimposed Lex’s face over the stunt double.

Honestly, her looking up made that shot 100% better. I’m glad they figure it out.
I’ve always regarded this moment as the most-anxiety inducing, intense scare of the whole movie. As a kid in the movie theater I’m sure I shrieked and instinctively raised my feet up onto my seat cushion to make sure no theater-raptors could get me.
In The Others, there’s a scene where the double of Nicole Kidman is running up the stairs, and the director (Alejandro Amenábar) told her to look up to the camera, because that way nobody would think it was a double.
While this is a very cool shot, it’s not true that it was to fix a mistake.
From Cinefex No. 55 Aug 1993 pg 93: “For Lex’s fall through the ceiling, a stunt girl stood in for Ariana Richards. ‘It was a ten-foot fall, so she was all wired up to avoid getting hurt. While Steven was rehearsing the action on the set, I got the idea of actually showing the real actress’ face for a second. Steven said, ‘Can you do that?’ And I said, ‘Yeah.’ So we shot the scene twice – once with a stunt girl, who did the fall and the subsequent struggling. We had her look up for a just a second.”
